A driver died in a Fontana crash this morning after swerving into a center median and striking two trees.

The unidentified motorist was driving a 2007 Toyota Tundra south on Liberty Parkway south of Meyer Canyon Drive at 12:26 a.m. The driver was thrown from the vehicle on impact, San Bernardino County coroner's officials said.

The driver was pronounced dead at 12:33 a.m.

Officials have identified the semi-truck driver who died Wednesday after a traffic collision in Fontana.

 

Rogelio Manuel Lopez, 22, of Mexicali, Mexico drove a truck carrying a load of steel, according to the San Bernardino County sheriff-coroner's Web site.

His truck was traveling westbound on the 10 Freeway east of Sierra Avenue about 1:30 a.m. when it collided with the rear of another semi truck, causing the cab of the rear truck to go under the trailer of the one in front, according to the Web site.

Lopez was pronounced dead at the scene, according to the Web site. The California Highway Patrol is investigating the collision.

A tractor trailer driver killed in a rear-end crash on the 10 Freeway this morning was struck by rebar and steel that shifted in his trailer, the California Highway Patrol said.

The 22-year-old man died at the scene on the westbound 10 east of Sierra Avenue at 1:26 a.m. His name was not available because his relatives had not been notified.

Officers said Yvonne Ashmore, 47, of Quitman, Texas, was in stop-and-go traffic with the hazard lights on her semi truck blinking.

The other driver didn't notice her in time and struck her. The impact caused the man's load to shift forward, officers said. The load punctured the driver's cab area, causing fatal injuries.

A semi truck driver died this morning when he rear-ended a big rig in Fontana, causing his cab to be crushed underneath the other driver's trailer.

San Bernardino County coroner's officials did not release the victim's name because his relatives have not been notified.

He was driving west on the 10 Freeway at 1:27 a.m. when he crashed into the back of another tractor trailer, coroner's officials said. The impact caused the victim's cab to go under the trailer in front of him. He was pronounced dead at the scene.

A Fontana man died early this morning after his vehicle struck a palm tree in San Bernardino.

Carl Marvin Raymondo, 46, was driving a 1993 Ford Thunderbird about 1:45 a.m. on Interstate 215, south of Highland Avenue, when the car  veered off the roadway and hit the tree, according to the San Bernardino County Sheriff's Coroner Web site.

The California Highway Patrol and San Bernardino City Fire Department arrived and pronounced  Raymondo dead at the scene, the coroner said.

A Fontana driver died earlier today during a freeway crash in Fountain Valley triggered when she dropped her cell phone and crashed her car while looking for it, authorities said.

The 33-year-old woman, identified by coroner's officials as Gladis A. Andrade-Zepeda, of Fontana, was driving her Chevrolet Impala on the 405 freeway just north of Bushard Street around 4:28 a.m. when she dropped her cell phone, California Highway Patrol Sgt. Dana Anderson said.

As Andrade-Zepeda was looking for the phone, her car swerved across lanes and hit the center divider, Anderson said. She and one of her other passengers - a 31-year-old woman - got out of the car.

It was completely dark because the car's headlights had been broken. A male passenger was still in the backseat.

Andrade-Zepeda tried to help the man get out, Anderson said. Suddenly, a Chevy Malibu came into the carpool lane and broadsided the Impala.

"They were unable to stop," Anderson said of the Malibu's two passengers, a male driver and a male passenger. "It appears they were going at a regular highway speed at the time."

Andrade-Zepeda was pronounced dead at the scene.

"Based on the severity of the injuries, it's quite possible she was entwined between both cars," Anderson said.

The men in the Malibu complained of pain and were taken to UCI Medical Center in Orange. Andrade-Zepeda's male passenger sustained moderate injuries and was taken to Long Beach Memorial Hospital. Zepeda's female passenger, who was standing by the center divider during the second crash, was not hurt.

Supervising Deputy Coroner Dan Aikin said an investigator reached Andrade-Zepeda's husband this morning and told him of his wife's death.

CHP is conducting the probe. The agency closed the freeway's northbound lanes until 7:15 a.m.

"Whether it's a cell phone, makeup or reading, especially when it's dark, drivers need to stop and pay attention," Anderson said. "Hands should stay on the steering wheel and a driver's attention should be on the roadway."
